How to Refer a patient for MFM Consult, Genetic Consult or Sonogram Only Services

Our New Referral form is as easy as 1-2-3.  Include your provider information, the patient information, and check off the service(s) request. Once your referral request is received, Eneida our referral coordinator, will contact your patient to schedule the appointment. Please fax all pertinent medical records and lab results to our office prior to the patient’s visit as well. We will take care of the rest!

Note: Many of our providers have EMR systems that autofax the request for consultation.  We welcome this!  If you do not use our Referral form, be sure to include all pertinent medical records with the request.  We will schedule the patient as soon as the complete record is received in these cases. 

Please be sure your patient brings a picture ID and insurance card with them on the day of their visit to our office. 

After Patient’s Appointment

After the patient’s appointment, you will receive a report from our office by fax, typically on the same day of service for MFM and Sono Only Services.   Our Genetic Consultation Report can take up to 4 business days to complete. 

Urgent Requests

Urgent Requests will be reviewed by the Physician and the patient will be contacted the same day the referral is received to let her know the status of her appointment with us.  All routine requests will be contacted within 24 hours of the received request.
